Pam Mouchka Family Memorial Scholarship in Physical Education & Health (CSH)

• completed three years at UW-L to be enrolled full time with one of the following major/minor combinations: double major in exercise & sport science and health; health major and exercise & sport science minor; or exercise & sport science major and health minor
• at least a 3.0 gpa
• involvement in extracurricular activities such as working with an athletic trainer or by doing some additional coaching
